Pennsylvania Health Insurance Plans For Participants
PA Health and Wellness offers Pennsylvania health insurance plans that fit your unique needs. Program eligibility depends on your age, income, family size and any special health needs you may have. These programs are part of Pennsylvania's Community HealthChoices program. Community HealthChoices is Pennsylvania's new program for providing long-term services and supports and home and community-based services, like services in nursing facilities and at home.
If you have Medicare, your health care benefits are not changing. If you also receive Medicaid long-term services and supports and want to enroll in PA Health and Wellness, you may consider learning more about Wellcare- our partner Medicare plan. However, you do not need to enroll in Wellcare in order to enroll in PA Health and Wellness Community HealthChoices.

Long-Term Services and Supports (LTSS): Community HealthChoices
PA Health and Wellness covers participants who are part of Pennsylvania’s Community HealthChoices program. This plan provides quality healthcare coverage with daily support and long term care services. Dual (Medicare and Medicaid) Special Needs Program (D-SNP): Wellcare D-SNP Medicare Advantage Plan
For people age 65 and older who are eligible for Medicare and Medicaid, Wellcare is our Dual Special Needs Program. Our Wellcare plans include comprehensive healthcare coverage with support you can count on. Please click on the logo for more information.
